What you need to do is start from scratch. A puppy will need to go to the bathroom 10 to 20 minutes after sleeping and after eating. The best way to potty train a dog is first dont free feed you dog (meaning dont put a big bowl of food down that your pup can eat from whenever he/she wants it) feed 2 to 3 times a day (depending on you puppys breed %26 or size). Then 10 to 20 minutes later take pup outside and wait for him/her to do its business. Sometimes it takes awhile for pup to go because he generally will take his time playing and sniffing for just the right spot be patient it will happen. When it finally goes then give lots of praise and affection so it knows its done something to please you.If you bring pup back inside too soon you will be cleaning up another mess. Take her back outside before bed time. Also if puppy is having trouble holding it all night then you need to consider crate training. Generally a dog wont go to the bathroom where he sleeps so get a crate just big enough for your pup to be able to stand up and turn around if its any bigger then he will go to the bathroom in the crate. Then first thing in the morning take pup straight outside to do her thing. Crate training is also ok while your at work (no longer than 8 hours) just leave some water and let her go to the bathroom before crating and when you get home. while you are at home its a good idea to put pup on a leash and tie it to your waist so everywhere you go he goes so when he starts sniffing around like hes looking for that perfect spot take him outside. Its always a good idea to pick ONE phrase or command when you take her out to go. Like: "take a break" or "go potty" so later she will know when and where its acceptable. Most importantly BE CONSISTANT otherwise you just confuse him and find yourself frustrated. I hope this was of some help to you.

Also female pups are notorious for pee'ing when their excited or being subserviant.
my friend dog is a bull and did that and she took it to the pet shop and asked what to do and they told her to buy a kennel and they dont go in the spot they lay and she takes the dog out to go potty and she lets the dog roam in the house for a certain amount of time and then put him in the kennel so it has bathroom time and realizes to wait till the bathroom time or scratch on the kennel and she or the kids let him outside and he got rewarded like a child for going potty outside verses inside
If she smells the area where she has been going, she will return to the same spot. Clean it with white vinegar, it will take out the smell and when you catch her going, tell her No. and immediately take her outside to go and when she does, tell her good girl and make a big fuss over her and reward her. She will eventually catch on.
LOL! That's how my dog was! I have a Chihuahua and she's 10 months old. Take him/her out almost every 25min. In the morning take your dog out and stay outside with it. and when he/she does something, give him/her a treat. that's what we did to our dog.

That is tough. My dog is 9 months and she has gotten good about whining to go out to potty but last week she did have a slip up. Have you tried spanking? I know it isn't fun for either of you but I don't know if dogs understand anything else that well.
You will have to crate the dog at night. That should be done from the beginning. Usually they won't use the bathroom where they sleep unless they are really small and the crate is big. You have to do this to train their bladders to hold and it takes time. So, you take the dog out before you go to bed and than crate the dog. As soon as you get up take it outside. This should produce something from the holding, if not put back in the crate and take outside again in an hour. Make sure the dog eats and drinks of course. This is how I have done it over the years. You have to be consistent like with kids and do it the same way all the time. If they go, get really excited, and praise the dog and clap your hands and give a treat. Scold the dog if you catch it doing it in the house. This has become habit and crating is the only way I know to break the habit, along with praise and being on the ball with it until it's working. Invest the time, it's worth it.
Be patient. Is your dog small? Because they are harder to train. It took my yorkie over a year to train properly, and she still has accidents occasionally.
You need to pay more attention to your dog. You need to take her outside every hour or two and praise her every time she goes to the bathroom. You can only expect a dog (AT MAXIMUM!!) to be able to hold its bladder its age in months +1. That means, at most, your dog can make it for 8 hours. My bet is that you are waiting too long.
Crate training will help, as others have mentioned since dogs will not pee where they sleep (unless of course you wait too long, no dog can hold it forever!).
Practice, practice practice. Everytime you go outside say go potty, go pee pee and PRAISE them when they do.
They get the idea - very quickly. but you MUST do this every hour/two since you have a dog that is peeing in the house.
If your dog is peeing too often, she may have an infection - see the vet.
Your dog may also be marking her territory/peeing where she has peed before. Mix up several cups of 50/50 vinegar and water and SOAK the carpet (don't do this on hardwood/pergo). Then cover with a towel and let it dry. The vinegar (acetic ACID) will destroy/denature the proteins in the urine reducing the smell and making it less likely they will pee there again
